I, the undersigned parent/guardian of the above-named participant, acknowledge that participation in the Dragon Wrestling camp involves inherent risks, including but not limited to, physical injury, paralysis, or death. I knowingly and voluntarily assume all risks associated with participation, whether known or unknown, even if arising from the negligence of the releasees or others.
I hereby release, waive, discharge, and covenant not to sue Augusta High School, the Augusta School District, tournament organizers, coaches, volunteers, officials, sponsors, and all associated personnel (collectively, the “Releasees”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by the participant or any personal property belonging to the participant, whether caused by the negligence of the Releasees or otherwise.
By acknowledging this page, I confirm that I am the legal parent/guardian of the participant and that I knowingly and voluntarily accept all risks associated with participation in the Dragon Youth Wrestling Camp, including but not limited to serious injury, paralysis, or death, even if arising from the negligence of the Releasees.
