    Pre-Packed meal bags are distributed to students on Thursdays at school to take home for the weekend. Please fill out and submit this form to start receiving weekend meals each week.   • Parental Consent Agreement for Weekend Backpack Meal Bags

    As the parent/guardian of the student(s) listed above, I hereby authorize the distribution of weekend backpack meal bags. I understand that by granting this authorization, my child/children will be responsible for bringing the meal bags home.

    The school will distribute elementary meal bags to each child's cubby, while middle school and high school bags will be provided to their 8th-hour teacher for the students to collect and take home.

    If a child does not take their bags home or is wasteful with food items—such as throwing food on buses or discarding items in the garbage—alternative arrangements for pick-up will need to be made. If these issues persist, meal bags will no longer be distributed to the child.

    Funding for this program comes from the generous donations of community members, and we want to ensure that the bags are being used appropriately.
