This is a student athlete and I understand that the WIAA requires all schools to provide transportation to and from athletic events in approved transportation.This is not a WIAA athletic event.
I certify that I am personally transporting the above-named student or have arranged for transportation with an adult (non-student) of my choosing that is not employed with the Augusta Area School District. I understand that transportation is being provided, but choose to provide my own child's transportatoin. By choosing to provide my own transportation, I am agreeing that I am in no way acting as an Augusta Area School District employee, agent, or volunteer. I also agree that the District is in no way hiring me, renting me, or borrowing my vehicle. I certifify that I agree to release, indemnify, and hold harmless the Augusta Area School District, employees, agents, and any and all persons' entities holding thereunder, including any and all policies of insurance from all liability or any adverse results that may occur.
